2024-2025 WELCOME BACK LETTER AND INSERTS
How to register your student at Oak Grove High School:
Print the required forms below
Fill them out, and bring them to the Registrar's Office at the school.
If you are unable to print the forms, copies are available at the school site.
If you have any questions regarding registering or withdrawing a student, please contact Mr. Lynch at 408-347-6514.
Read the handout regarding the language acquisition program we offer our English Learners.
Required Forms:
English: Enrollment Form, Emergency Contact Form, Language Acquisition Program
Español: Forma de Inscripcion, formulario de contacto de emergencia, Programas de Adquisicion de Lenguage Inglés
Tiếng Việt: Mẫu đăng ký, Mẫu liên hệ khẩn cấp, Chương Trình Hấp Thụ Ngôn Ngữ
Additional paperwork needed for registration:
All registration papers must be filled out and signed by a parent or guardian
Bring:
Legal papers designating a non parent as guardian if applicable.
Copies of the withdrawal papers and transcripts from the school your student attended before enrolling here
Copy of the student's immunization records
Tuberculosis test that is no older than 12 months (non Santa Clara County residents only)
Chicken Pox vaccination record or doctor's note stating the student had Chicken Pox as a child (non California residents only)
Address verification for Oak Grove's attendance area.
PG&E bills, water bills, credit card bills, bank statements, mortgage papers and apartment rental agreements.
The bills must be in the parent's or guardian's name.
The following can not be accepted:
personal checks, driver's licenses or phone bills, bills older than 1 month.
Current IEP for Special Education Students
If a student qualifies via the McKinney Vento Act, some of these requirements do not apply.
